Information for Youth Risk Behavior Survey

Wednesday, January 9, 2019

Hi OHS Families,

As we prepare students to be career, college, and life ready, it is important we offer a variety of learning opportunities and programs to not only support the academic development of each child, but also to guide their social and emotional development. In order to inform future programming and to identify trends among our students, we will once again administer the Youth Risk Behavior Survey (YRBS) to all of our students.

The survey has been designed by the US Centers for Disease Control and will be administered during a homeroom period on Monday, February 4, 2019. The survey will collect information on the challenges and issues impacting our youth. Topics include:
  • Health, nutrition, and safety behaviors
  • Emotional and mental health
  • Sexual activity
  • Drug and alcohol use
  • School experience
  • Peer relations and bullying
  • Social networking
  • Community relations and support

The survey is anonymous and the data will be used to identify trends to help inform us what programs and services we need to provide to support the social and emotional development of the students in the OHS community. In addition, the survey is completed online and the information is tabulated by the Wisconsin Department of Public Instruction, so that no one in our community will be aware of any individual student responses. We have conducted this survey in the past at both the high school and intermediate level, and our team has benefited greatly from the data.

Please note that in accordance with our district policy and the Family Educational Rights and Privacy Act, parents/guardians have the right to review the survey prior to its distribution. The survey is available in the Student Services office at Main Campus. You are welcome to stop by during our regular office hours of 7:00am-3:00pm or call Kris Zimmer at 262-560-3130. Parents and guardians also have the right to exempt a child from participation. To do so, please print, sign, and return the  exemption form at the end of this message to Kris Zimmer in the Student Services office or scan a signed copy and send to ZimmerK@oasd.org by January 28, 2019.

Also, attached to this message is a series of Frequently Asked Questions regarding the survey. If you have any additional questions or specific concerns, I would encourage you to reach out to School Counselor, Scott Bakkum at BakkumS@oasd.org or by phone at 262-560-3123 or School Psychologist Brianne Decker at DeckerB@oasd.org or by phone at 262-560-3126.

Thank you for your help as we work to support our students during this exciting yet challenging time in their lives. Feel free to contact me at CurtisJ@oasd.org if you have any other questions.

Jason
